Our four member practices care for a population of approximately 50,747 patients.

Services include:

​Social Prescribing Link Workers & Care Navigators

Our Social Prescribing Team will support you holistically by connecting you to non-medical support in and around the community. Social prescribing helps those with a wide range of social, emotional or practical needs, and often focuses on improving mental health and physical wellbeing.

Patients who could benefit from social prescribing schemes include; people with mild or long-term mental health conditions, people with complex needs, people who are socially isolated, and those with multiple long-term conditions who frequently attend either primary or secondary health care.

First Contact Physiotherapy

We have a primary care based Physiotherapy service which allows patients, who are contacting their GP surgeries with musculoskeletal pain to be seen quickly by a specialist musculoskeletal physiotherapist. The physiotherapists’ will assess, diagnose and provide information on appropriate individualised rehabilitation and management plans. They will work closely with our clinicians to ensure patient’s get the best level of care and assist with any medication, investigations and referrals when deemed necessary.

This service is provided by Pure Physiotherapy and is supported by our local GP Federation One Health Lewisham.

You can book an appointment directly with a physiotherapist by speaking to the reception team at your GP practice or by downloading the ASK NHS APP.

Health and Wellbeing Coach

We have recently employed a Health and Wellbeing Coach who works across all four of our member GP practices. Health and Wellbeing coaches use a variety of health coaching techniques to engage patients and support them by playing an active role in their health. They are able to support patients with a series of coaching sessions focusing on a variety of issues including:

  • Support to live a healthy independent life
  • Achieving a healthy weight
  • Support for patients with Type 2 Diabetes
  • Improving low mood/ mental health support (low-level)

​Extended Hours

​GP practices are working together to offer patients better access to GP appointments and to provide additional appointments outside of their normal opening hours. These are known as Extended Hours appointments and are available during Monday to Friday; early morning or late evening. Please contact your local GP practice or visit your practice website for further information. 

If you or a member of your family need medical advice or treatment and/or need to see a GP outside of normal GP practice hours, you may be offered an appointment as part of our Enhanced Access Service.

What is Enhanced Access?

The Enhanced Access Service provides patients with additional appointments outside of normal GP opening hours. These are known as enhanced access appointments and are available

  • Monday-Friday: 18:30 to 20:00
  • Saturday: 09:00 to 17:00

Please note that this is not a walk-in service.

Appointments are available every Saturday. Your practice will offer you a Saturday appointment on the scheduled days they are open.

Who can use Enhanced Access Appointments??

The appointments are available to patients who are

  • Registered with a GP practice within Aplos Primary Care Network (PCN) About Us – Aplos Health PCN
  • Your appointment request has been assessed and triaged by your own GP practice and subsequently booked you an enhanced access appointment. 
  • Routine appointments as identified by your GP.

Some of our sites also offer appointments for some of the following services;

  • Routine GP appointments
  • Urgent GP appointments
  • Routine Nursing appointments e.g. injections, wound dressings
  • NHS Healthchecks
  • Diabetic Foot Checks
  • Child Immunisations
  • Phlebotomy / bloods
  • Cervical Screening /Smear Tests
  • Contraception advice and Implant /Coil Fitting
  • Medication review

 Will I be seen face to face?

Face to face appointments are available, although you may also be offered a telephone consultation.

Can I ask for an enhanced access appointment?

If you are unable to get an immediate appointment at your GP practice, an enhanced access appointment should be offered to you. If this is not offered, please ask the receptionist.

What if I need to cancel my appointment?

Please contact your practice to cancel or rearrange your appointment.

Care Home and Residential

We provide care and support to Peartree care home, as well as several smaller learning disability and mental health homes. We are also working with a new One Health Lewisham hosted Care Home Team to enhance the current provision.
